Hollywood Cemetery Permit Application

Note: All grave sites MUST be marked prior to work beginning. Head stone size is restricted to 6’ (72”) high maximum, measured from the ground elevation of the lot. Benches and other fixtures are prohibited. Do NOT install permanent fixtures until you have become familiar with the Rules and Regulations of Hollywood Cemetery, and obtain the required permit. Failure to abide by these regulations may require alteration of installed head stones and other fixtures; or their removal at owners expense. A drawing of proposed work – including dimensions of proposed installed head stones and fixtures – MUST be included. You should allow a minimum of 24 hours to obtain permit approval. All monument installations/head stones require a footing, and a footing inspection.
